Position Summary:

The community telemedicine project works with clinical health providers in community clinics, community-based organizations providers located locally and off-site. The goal of the program is to enhance primary and specialty healthcare through a telemedicine platform to increase healthcare access in Cameron County, with a particular focus, but not exclusively, on uninsured residents. The RA will be responsible for interacting with providers and patients to address barriers to engaging in primary and specialty telemedicine consultations, and assisting in follow up care. The RA will consent the patient, assist with appointment setting, conduct home visits and assessments as needed prior to the telemedicine visit as well as follow up visits as necessary. The RA will be the liaison between the healthcare team and the patient, and will assist the patient with technology, with connection, preparation for visit, education, and follow-up care. The RA may also assist the provider in conducting basic physical assessments, utilizing a wireless blood pressure cuff or manipulating a wireless stethoscope. The RA will also address community issues and other social determinants of health as needed under the direction of the provider. The information gathered at the home visits will be documented in a HIPAA protected cloud-based data system and/or Electronic Medical Record along with instructions for follow up care or other issues identified by provider and patient.

Position Key Accountabilities:

  • Under close supervision, conducts a range of activities according to protocol. Activities may include some or all of the following: contacting and working with patients to initiate telehealth consults and coordinating with providers. Home Visits: Conduct home visits with patients to teach and prepare them for telemedicine visit. Assist patients with logging onto system, downloading the application, including conducting some potential screenings (HbA1c), BMI, PHQ9, Medication Adherence, assisting the provider in basic measurements using blue tooth or USB instruments for assessments such as blood pressure, or stethoscope transmitted directly to the provider. Work with provider and patient to prepare questions for provider follow up, and to address lifestyle issues such as physical activity, nutrition and social determinants of health issues.

  • Manages telemedicine platform (virtual) and stay abreast of changes in order to teach patients how to use the system.

  • Maintains clean equipment and/or work area to ensure a safe and efficient work environment.

  • Reviews patient assessment and notes: Prepare notes and assess needs. Discuss and prepare presentation on cases for review to supervisor and health provider team.

  • Data System: Complete assessments, case notes and questionnaires accurately in program data system (EMR) in coordination with provider.

  • Stays abreast of new and updated protocols for clinical and research activities.

  • Complies with all governmental and University policies, rules, regulations, and codes.

  • Performs other duties as assigned.

Certification/Skills:

  • General knowledge of research fields.

Preferred:

  • Bilingual in English/Spanish

  • Community Health Worker/Case Management or Health Education experience preferred

  • CHW DSHS certification or willingness to obtain within a year of employment

  • Willingness to work some evenings and weekends to accommodate participant schedules

  • Experience conducting surveys or questionnaires preferred

  • Computer skills including Excel, familiarity with video conferencing platforms like Zoom and Webex

  • Must be able to travel to off-site locations alone to complete work assignments.

  • Current CPR/First Aid certification preferred or willingness to obtain within 3 months of employment

Minimum Education:

  • Bachelor’s degree or relevant experience in lieu of education.

Minimum Experience:

  • No years of experience.

Physical Requirements:

  • Exerts up to 20 pounds of force occasionally and/or up to 10 pounds constantly and/or a negligible amount constantly to move objects.

  • This position may include work involving potentially hazardous chemical, biological or radioactive agents.
