UTHealth - UT Physicians Multispeciality Clinic - Rosenberg (5115 Avenue H, Rosenberg, Texas) is hiring a CERTIFIED Community Health Education Specialist (CHES) to join their team! This position works with individuals, families, and communities, as well as public and private organizations to create, implement, oversee, and analyze programs and strategies that promote health and well-being. Ideal candidate will be bilingual (English/Spanish) with experience working with community agencies and health related institutions. Knowledge of Rosenberg and surrounding Southwest Houston area is a plus!

Position Key Accountabilities:

  1. Conducts needs assessment for clinic/community and introduce new wellness programs based off of needs assessment. This includes collecting and analyzing clinical and community based data and researching interventions.

  2. Assists with cultivating new partnerships with community groups and organizations that benefit the clinic and patient population.

  3. Attends community meetings and/or serves on community boards to understand needs and improve health outcomes of patient population.

  4. Participates in community events to provide health education.

  5. Works collaboratively and understands the roles of providers, medical assistants, nurses, referral coordinators, clinical pharmacists, diabetes educators, social workers, community health workers, and other clinical staff to ensure comprehensive health promotion.

  6. Prepares, coordinates, develops, implements, and evaluates evidence-based health education/promotion programs in-person or virtually . This may include identifying potential partnerships and/or developing curricula driven by research.

  7. Develops health education materials .

  8. Assists in the writing, editing, and reviewing of various publications, newsletters, and catalogs.

  9. Provides brief health education counseling , on various health topics, face-to-face or over the phone to build rapport with patients and recruit for programs.

  10. Documents patient encounters and participation in wellness programs in electronic health record and reports program outcomes quarterly.

  11. May perform other duties as assigned.


Certified Health Education Specialist (CHES ®) or obtain certification within 6 months of hire.

Minimum Education:

Bachelor’s degree in Health Education, Community Health Education, Public Health, School Health Education, or degree/major in a discipline of "Health Education”

Minimum Experience:

Experience working with community agencies and health related institutions / Experience in implements wellness programs.

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
