Position Summary:

Responsible for leading operational and clinical integrity of multidisciplinary care teams for UT Physicians. Will provide oversight to patient safety and quality initiatives, development and implementation of and adherence to protocols and processes, including data integrity.

Position Key Accountabilities:

  1. Leads, directs, manages care team leadership and staff in their daily operations, and develops and implements systems for optimal knowledge transfer and efficiency across the practice.

  2. Serves as primary contact and resource for team-based approaches, performance, and activities aligned with the Chronic Care and Patient-Centered Medical Home Models. Tracks and reports on the progress of assigned clinical initiatives, including budget and timelines.

  3. Responsible for developing new care management programs of a clinical nature and providing oversight and evaluation of existing programs.

  4. Strategically sets goals and directs the design and implementation of processes aligning the quality improvement across all operational sites to meet strategic goals.

  5. Collaborates with the system, regional, and campus leadership to support institution-wide initiatives, including continuous improvement programs, ambulatory programs, regulatory readiness, and high-reliability strategies.

  6. Directs the setting and updating of clinical policy and procedures, provides guidance to care team leads making clinical judgments, creates coverage plans for the care team, and develops daily operational leaders to build clinical services strategies.

  7. Evaluates care management staff performance by providing monthly management level and role level report cards, including developing improvement plans for poor performance.

  8. Assist senior leadership with long-term planning initiatives to maintain operations assuring activities are appropriately integrated into the strategic direction and the practices mission and values.

  9. Manages Human Resources activities for direct reports regarding recruiting and selection; hiring and termination, training, development, mentoring, counseling, performance evaluations, and salary planning.

  10. Coordinates duties with appropriate personnel to meet operational program needs, and ensures compliance with state and federal health plan requirements, Medicare guidelines, and URAC/NCQA standards.

  11. Performs other duties as assigned.


Current Texas Licensure as a Registered Nurse

Project Management and/or Six Sigma certification preferred

Excellent written and verbal skills

Minimum Education:

Bachelor’s degree in Nursing from an accredited nursing school

Master’s degree in Public Health or Healthcare Administration preferred

Minimum Experience:

Six years of experience in clinical operations to include experience in case management, project management and quality improvement.

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up-to 10 pounds frequently and/or a negligible amount constantly to move objects.
